[XEN][PAE] Always enable non-debug version of l3tab_needs_shadow()

[XEN][PAE] Always enable non-debug version of l3tab_needs_shadow()
as there is some suspicion that it leads to crashes on PAE hosts.
Also: improve bracketing in the macros, for safety.
